We are seeking a dedicated and compassionate Case Manager to join our client’s team in the healthcare industry. The Case Manager will be responsible for assessing clients' needs, developing care plans, coordinating services and monitoring progress towards goals. The ideal candidate will have excellent communication and organizational skills, a strong understanding of social services and the ability to work effectively with diverse populations.

The Case Manager will primarily work in an office setting but will also travel to clients' homes, healthcare facilities and community organizations as needed. This position may require occasional evening or weekend hours to accommodate client needs or attend meetings.

Case Manager Responsibilities:

  • Conduct initial assessments to identify clients' strengths, needs and goals
  • Develop individualized care plans in collaboration with clients, families and other stakeholders
  • Coordinate services and resources to meet clients' needs, including healthcare, housing, employment and social support
  • Advocate for clients' rights and access to services within the community
  • Monitor and evaluate clients' progress towards goals, adjusting care plans as needed
  • Provide crisis intervention and support as necessary
  • Maintain accurate and up-to-date documentation of client interactions and service provision
  • Collaborate with interdisciplinary teams, including healthcare providers, social workers and community organizations
  • Participate in ongoing training and professional development activities
  • Stay informed about relevant policies, regulations and best practices in the field of case management

Case Manager Requirements:
  • Bachelor's degree in social work, psychology, counseling or a related field
  • Previous experience in case management or a related field
  • Strong assessment, problem-solving and decision-making skills
  • Excellent communication and interpersonal skills
  • Ability to work independently and as part of a team
  • Knowledge of community resources and support services
  • Experience working with diverse populations, including individuals with mental health issues, substance abuse disorders and/or disabilities
  • Familiarity with electronic health records and other documentation systems
  • Commitment to maintaining confidentiality and respecting clients' autonomy and dignity
  • Valid driver's license and access to reliable transportation (if applicable)
  • Must be able to sit, stand, walk and lift objects up to 20 pounds
  • Ability to communicate effectively both verbally and in writing

Case Manager Benefits:
  • Salary for this position will be commensurate with qualifications and experience, between $50,000-$56,000
