LaSalle Network is hiring a Care Coordinator Supervisor to support and lead care coordination services for aging and vulnerable populations across Cook and Lake counties. This Care Coordinator Supervisor role is ideal for a compassionate and experienced leader who enjoys guiding teams, strengthening client outcomes, and ensuring high-quality, compliant service delivery in a mission-driven environment.

The Care Coordinator Supervisor will oversee Care Coordinators and support staff while also providing direct service to clients with complex needs. This role requires strong leadership, excellent communication skills, and the ability to balance supervision, compliance, and hands-on client support in a fast-paced environment.

Responsibilities:
  • Provide direct supervision, coaching, and support to Care Coordinators, volunteers, interns, and case support staff
  • Deliver direct services to elderly and frail clients with complex care needs
  • Assign work, provide training, and ensure staff performance meets quality and compliance standards
  • Resolve complex client concerns and coordinate responses with community partners
  • Oversee accurate and timely documentation, including client files and databases
  • Monitor compliance with program guidelines, policies, and reporting deadlines
  • Complete monthly reporting and assist with data collection, analysis, and performance tracking
  • Travel throughout assigned service areas to support staff and client needs
  • Collaborate with internal teams and external partners to ensure coordinated service delivery
  • Foster a positive, accountable, and mission-driven team culture
  • Support staff development through feedback, training, and performance management
  • Contribute to continuous improvement initiatives and service enhancements

Requirements:
  • Bachelor’s degree in social work, human services, healthcare administration, or related field preferred
  • Previous supervisory or lead experience strongly preferred
  • Experience in care coordination, case management, or aging services required
  • Strong communication, leadership, and conflict resolution skills
  • Experience with data entry and case management systems
  • Ability to manage multiple priorities and maintain accurate documentation
  • Valid driver’s license and willingness to travel locally for client visits
  • Experience in aging network services, federal waiver programs, or managed care preferred

Key Details:
  • Compensation: $50,000-$54,000 annually, depending on experience
  • Benefits: Medical, dental, and vision insurance available
  • Location: Matteson, IL and traveling through Cook and Lake County, IL (field-based with administrative responsibilities)
  • Work Schedule: Monday-Friday, 8:30 AM – 4:30 PM, on-site + in field days
